Welcome to the Louisville Box Classic Lacrosse Tournament hosted by L4 Lacrosse! We are excited to have you and your team participating in our event. Before you can take part in the tournament, we kindly ask that you carefully read and acknowledge the terms of this Waiver of Liability and Release Agreement.

1. Assumption of Risk: I understand and acknowledge that participating in lacrosse involves inherent risks and dangers, including but not limited to physical injury, illness, or property damage. I voluntarily assume all such risks associated with my participation and the participation of my team in the Derby City Clash Box Lacrosse Tournament.

2. Release of Liability: I hereby release, discharge, and hold harmless L4 Lacrosse, its organizers, sponsors, officials, volunteers, and any associated parties from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of any loss, damage, or injury, including death, that may be sustained by me, my team members, or any property while participating in the tournament.

3. Insurance Coverage: I acknowledge that it is my responsibility to have adequate insurance coverage for any injuries or damages that may occur during the tournament. L4 Lacrosse does not provide insurance coverage for participants.

4. Medical Consent: In the event of an emergency, I authorize L4 Lacrosse and its representatives to secure any necessary medical treatment for me or my team members. I understand that any medical expenses incurred will be my responsibility.

5. Photography and Media Release: I grant permission for L4 Lacrosse to use photographs, video recordings, or other media taken during the tournament for promotional purposes without compensation.

By signing below, I acknowledge that I have read, understood, and voluntarily agree to the terms and conditions outlined in this Waiver of Liability and Release Agreement.
