Waiver of Liability & Release Agreement

Welcome to the JV Cup Lacrosse Tournament hosted by L4 Lacrosse LLC. We are excited to have you and your team participating. Before taking part in the event, please read and acknowledge the following terms of this Waiver of Liability and Release Agreement.

Assumption of Risk

I understand and acknowledge that participating in box lacrosse involves inherent risks, including but not limited to physical injury, illness, or property damage. I voluntarily assume full responsibility for any risks associated with my participation and the participation of my team in the Barrel 6s Lacrosse Tournament.

Release of Liability

I hereby release, discharge, and hold harmless L4 Lacrosse LLC, its directors, organizers, staff, coaches, officials, volunteers, sponsors, and all associated parties from any and all liability, claims, demands, actions, or causes of action arising from any loss, injury, damage, or death that may occur to myself, my team members, or our property while participating in the tournament.

Insurance Coverage

I acknowledge that I am solely responsible for securing and maintaining adequate medical or personal insurance coverage for myself and my team members. I understand that L4 Lacrosse does not provide insurance coverage for participants.

Medical Consent

In the event of an emergency, I authorize L4 Lacrosse LLC and its representatives to obtain or administer any necessary medical treatment for myself or my team members. I understand that all related medical expenses will be my sole responsibility.

Photography & Media Release

I grant permission to L4 Lacrosse LLC to capture, use, and publish photographs, video, or other media of myself or my team during the tournament for promotional purposes without compensation.

Acknowledgment & Agreement

By signing below, I acknowledge that I have read, understand, and voluntarily agree to the terms and conditions outlined in this Waiver of Liability and Release Agreement.
